What are the various forms for PF?

Form 2 (Revised): To be submitted by member for nomination.

Form 5(IF): In the event of death of member, this form is to be used by a nominee / legal heirs for settlement of Employees’’Deposit Linked Insurance Fund.

Form 10-C:

  1. To be submitted by member for withdrawal (in case of members with less than 10 years of membership).
  2. To be submitted by member for availing Scheme Certificate (in case of members with less than 10 years of membership and or not using above two options).
  3. To be submitted by nominee / parent for settlement of Lump sum Benefit of deceased member (who had no family and died when not in service in covered establishment).

Form 10-D:

  1. To be submitted by member / nominee for availing pension in case of membership of more than 10 years in EPS and attained the age of 50 for Reduced Pension and 58 for Superannuation Pension.
  2. To be submitted by family member for availing pension in case of member died in service/while holding Scheme Certificate and having a family.
  3. To be submitted by nominee for availing pension in case of member died while in service and nominated any person other than the Family entitled for pension.

Form 13 – For transferring the Provident Fund account of a member from one establishment to another establishment covered under the act / scheme.

Form 14 – Application for financing a life insurance policy out of Provident Fund account.

Form 19 – To be submitted by a member to withdraw his/her Provident Fund dues on leaving service or retirement or termination.

Form 20 – In the event of death of member, this form is to be used by a nominee / family member to claim the member’s Provident Fund accumulation.

Form 31 – For the use of Provident Fund members to avail advances / withdrawals as provided in the scheme.

    Res. Madam,
    I would like to have an official copy of Application form for pensioner for revision of pension scheme as per the reference pen – I /12/33/ EPS Amendment/96/VOL. I dated 23-03-2017. Of EPFO headquarters.
    Kindly forward me the same to me for applying.

  2. I have received the PF amount in my Account but it is less and I have not received the scheme certificate for pension as I had been working in the organization for 23 years

    Dear sir,

    I ‘m saritha Reddy, resident of hyderabad, i just need a small information regarding my provident fund claim no.KDMAL02124800000046533, which i have worked in FUTURE VALUE RETAIL LTD, Hyderabad, from 2012 to 2013, I have resigned for the job due to personal reasons. and i didn’t apply for PF settlement that time , and Now I want to settle my PF account , So, I would like to know that, Can i send my application form through post to your EPF organisation kandivali, address and does it require 30 rupees affidavit , along with my PF application form,becoz almost two years passed after quitting my job. So, Plz kindly give information regarding above issue, i expect a prompt reply .
