On 8th April, 2019 I have sent my pension papers to you through my employer, bajaj electricals ltd., alongwith original Scheme Certificate which is mentioned above, but regret to inform you till date my pension has not been started, although more than 4 months passed. I am member of family pension scheme 95 & my UAN No: 100382594127 have claimed as per Form 10 D of family pension on dated 02.09.2021. I had been attached Form No.10 D , Bank Pass Book & cancelled cheque to your office. When i have checked claim status that status are showing your claim settled on dated 05.10.2021.
